For your convenience, we have made a number of forms available for our patients to download. Please print and complete the following forms and bring them to your visit: Patient Consent, History, and Payment Policy. Having these forms ready prior to your visit will expedite your check in process. If you wear Contact Lenses or are interested in them, also complete the CL Policy.

We have also included some educational forms about how to care for and wear your contact lenses. If you sleep in contact lenses, be sure to view the EW (extended wear) contact lens guidelines.
